Please complete the below form and submit.  Once received an invoice will be sent via email with the option to pay online via credit card.  A W-9 will be required by the RMMRA prior to payout.

Please download, print and return via USPS with a check in the amount required.  If this method is desired then please include W-9 along with the application.

Please Download the attached W-9. Print and bring a completed copy to your first race or mail in with paper application. Payouts will not be made until the form is received by RMMRA.
